Implementing, and Evaluating Entrustable Professional Activities (EPAs) in Medical Education

The goal of CBME is to cultivate competent healthcare professionals who can provide medical care according to local contexts, achieving a level of professional proficiency defined by local needs. The implementation of CBME curriculum primarily involves three aspects: outcome-oriented competency frameworks and progress descriptions, teaching and learning experiences tailored for competencies, and programmatic assessment design and competency progress evaluation. Internationally, the main models for describing competency frameworks and progress are milestones and Entrustable Professional Activities (EPAs). Both milestones and EPAs operationally define the “competency progress”. Milestones start by describing the “performance characteristics” of individuals, with the progress of each competency clearly visible in the descriptors used at different levels. EPAs, on the other hand, focus on describing the “professional tasks that can be entrusted” to an individual. Many professional societies currently choose to use the six core competencies as a framework, using milestones or EPAs, or a combination of both, to depict the progress of core competencies, forming a concrete and operational blueprint for professional development. This presentation will share the state-of-the-art of developments in medical education domestically and internationally, common challenges in implementing CBME, possible implementation strategies and the development of EPAs.

勝任能力導向醫學教育(CBME)目的在于培養出勝任的健康照護專業人員,能依據當地情境,執行醫療照護,達到符合在地需求所定義的執業精熟度。推行CBME主要有三個大面向,分別為成果導向的能力架構與進展描述、為勝任能力量身打造的教學與學習經驗、計畫性與系統性的評量設計與能力進展評估。而描述能力進展架構的模式,國際間主要有能力里程碑milestones與可信賴專業活動EPAs兩種模式。milestones與 EPAs皆是操作型定義「能力進展」的方式,milestones 由描述「人的表現特質」之進展著手,每項能力的進展,可以由不同階層的milestones描述所用的形容中清楚看出;而EPAs則聚焦描述人所能「被信賴去做的專業任務」。當前許多的學會專業團體,都選擇以六大核心能力為框架,用milestones或EPAs,抑或兼用之,來描繪出核心能力的進展,構成整個專業養成具體可操作的藍圖。本演講將分享國內外醫學教育的最新進展,推行CBME常見的挑戰,實施的可能策略以及EPAs發展。
